What Are the Main MSW Specializations, and Which One Fits the Work You Want?
The main MSW specializations are clinical and mental health, child and family, macro practice, medical social work, school social work, aging and substance use, and 336 of the 530 MSW programs on this site name at least one. Clinical is by far the most common, offered by 256 programs at 207 schools, while school social work (39 programs) and aging (35) are the rarest. Your specialization shapes your second-year courses and field placement; it does not change which license you can apply for.

How MSW Specializations Work
An MSW has two layers. The first is generalist practice, which every student takes. CSWE’s 2022 accreditation standards (EPAS) describe it as practice at the micro, mezzo and macro levels, built around nine competencies such as ethical and professional behavior, policy practice, and engaging, assessing, intervening and evaluating with individuals, families, groups, organizations and communities.
The second layer is specialized practice. CSWE requires every accredited MSW to have at least one area of it, and that area extends the same nine competencies to a particular population, problem or method. That second layer is what schools call a concentration, specialization, focus area or track.
In practice, the specialization decides three things: your advanced courses, your second field placement and the kind of supervisor you’ll train under. With a CSWE-accredited BSW, you usually skip the generalist year through advanced standing and start in the specialization, so the choice comes sooner.
Of the 530 MSW programs verified on this site, 336 name at least one specialization. Many list several, so a single program can count toward more than one row below.

Three things stand out. Clinical is offered almost everywhere, in 46 states, so you can usually find it close to home. Aging has the lowest ceiling: its highest estimate is $75,000, while every other specialization’s highest estimate is $87,540 or more. And the medians are tighter than the ranges suggest, sitting between $39,000 and $47,700 for all seven, so the school you pick moves the price far more than the specialization does.
Program structure barely changes between specializations. The median program is 60 credits with 900 field hours in every row, and the median advanced standing plan runs 30 to 34 credits.
Clinical and Mental Health: The Masters in Clinical Social Work
A masters in clinical social work is an MSW with a clinical or mental health concentration. It’s the most common choice because it lines up with the clinical license. BLS describes clinical social workers as professionals who diagnose and treat mental, behavioral and emotional problems, and says they need a master’s degree, supervised clinical experience and a state license.
Program names vary. Kennesaw State University’s MSW calls it Clinical social work practice. Lewis University splits advanced clinical practice by population: child and adolescent, adult, and older adult. East Carolina University’s online MSW frames it as Clinical-Community Social Work Practice from a Relational Perspective.
Of the 256 clinical programs, 238 offer advanced standing and 169 list a part-time plan. 224 are CSWE-accredited and 32 are in candidacy or pre-candidacy.
How to Become a Licensed Clinical Social Worker
The clinical license comes from your state board, not your school. The order is broadly the same everywhere, though hours and tiers vary by state:
- Earn an MSW from a CSWE-accredited program, which most state and territory licensing rules require.
- Get your state’s master’s-level license, usually after the ASWB Masters exam.
- Complete your state’s supervised clinical practice.
- Pass the ASWB Clinical exam.

Child, Family and School Social Work
These two specializations often overlap. BLS says child and family social workers protect vulnerable children and help families in need of assistance, while school social workers develop plans to improve students’ academic performance and social development. BLS counts both in one occupation.
Child and family is the third-largest specialization, with 112 programs. Names include Family & Child Welfare at Alabama A & M University, Children, Youth, and Families at Southern University at New Orleans, and Specialized clinical social work practice (children, families and vulnerable adults) at Albany State University. Its median estimated tuition of $47,700 ties aging for the highest of the seven.
School social work is small: 39 programs in 15 states. It carries an extra step: school jobs can require an education credential on top of the social work license. Governors State University names this directly: its School Social Work Practice concentration is built around the Illinois Professional Educator License (PEL). Medical Social Work and Aging
Medical social work puts you in hospitals, clinics and hospice. BLS says healthcare social workers help clients understand their diagnosis and adjust their lifestyle, housing or healthcare. 67 programs at 57 schools offer it, and it has the lowest median estimated tuition of the seven at $39,000.
Some programs merge medical and clinical work: the University of Alabama at Birmingham lists Clinical/Medical social work (health and behavioral health), and Florida Gulf Coast University pairs Clinical practice with a Medical Social Work Concentration.
Aging (gerontology) is the smallest specialization, with 35 programs in 18 states. Its estimates top out at $75,000. George Mason University’s online MSW calls it Adults & Healthy Aging; California State University-Long Beach calls it Adulthood and Aging; and Bowling Green State University runs it as Advanced Generalist: Gerontology. If you want to work with older adults, a medical social work concentration is a close alternative, with 67 programs to choose from.

Macro Social Work: Policy, Communities and Leadership
Macro level social work means changing systems rather than treating individuals: running programs, organizing communities and shaping policy. It’s the second-largest specialization, with 142 programs in 39 states.
Concentration names vary, and many skip the word “macro”; the University of North Carolina Wilmington is one that uses it, next to Direct Practice. Appalachian State University’s online advanced standing MSW calls it Community and Organizational Practice. The University of Illinois calls it Leadership and Social Change. Savannah State University offers Social Administration, and the University of Arkansas at Little Rock offers Social and Organizational Impact, on the main campus only. Northeastern State University in Oklahoma focuses on Indigenous & Rural Communities.
Mezzo Level Social Work
Mezzo sits between the two: work with groups, organizations and neighborhoods. You’ll find mezzo work inside both clinical tracks (group therapy) and macro tracks (organizational practice). A concentration such as Clinical-Community practice or Integrated Clinical and Community Practice covers both ends.
Substance Use and Narrower Concentrations
Substance use is offered by 29 programs at 27 schools. 25 of those programs also carry a mental health tag, so most treat addictions as part of clinical work rather than apart from it. The substance use MSW programs page compares them.
Beyond the seven main groups, schools offer narrower tracks: forensic social work (Northern Kentucky University), military and veteran social work (Aurora University), international social work (Metropolitan State University of Denver) and bilingual/bicultural clinical practice (New Mexico Highlands University). These suit you if you already know your setting. They can also narrow your placement options, so weigh them against a broader concentration with an elective.
What Each Specialization Pays
Pay follows the job, not the concentration name. BLS groups social workers into four occupations, and each specialization lines up most closely with one of them. Figures are May 2025 national medians.

How to Choose an MSW Specialization
Work through these in order:
- Picture the setting first. Hospitals point to medical social work, schools to school social work, treatment programs to clinical or substance use, agencies and government to macro.
- Check whether the setting needs an extra credential. School jobs may need an education license, as Governors State’s PEL-linked concentration shows.
- Count your options. Clinical is in 46 states; school social work is in 15. If you need a campus program nearby, a rare specialization may not be close.
- Check the format. Only 15 aging programs and 20 school programs run online, against 129 clinical programs.
- Compare price at the school level. Medians run only from $39,000 to $47,700 across specializations, but the range within each is wide.
- Check the second placement. Your specialization-year placement is where you build your first professional network. Look for schools that name placement settings in your area.

Does Your Specialization Limit Your License?
No. State boards license you by degree level and exam, not by concentration. ASWB’s four exam categories are Bachelors, Masters, Advanced Generalist and Clinical, and most states offer several license tiers with requirements that vary by state. A macro MSW graduate can sit for the same master’s-level license as a clinical graduate.
The practical limit is experience. A clinical or independent license requires supervised practice after the degree, and a clinical concentration gives you a clinical placement and clinical coursework to start from. If you might want an LCSW someday, choose a clinical concentration or a school that lets you add clinical electives to a macro track.
The other trade-off is accreditation. 49 programs nationally are in CSWE candidacy rather than accredited, and CSWE notes that most states require an accredited degree for licensure. Check a program’s status on its profile before choosing it for a specialization no one else nearby offers.

Do You Have to Choose a Specialization in an MSW?
Every CSWE-accredited MSW must teach at least one area of specialized practice, so you study one even if the school calls it advanced generalist. Some programs, such as Ball State University's, offer a single advanced generalist track instead of a menu.
Can You Change Your MSW Specialization Later?
Your license is based on your degree level, not your concentration, so a macro graduate can still move into clinical work. For a clinical license, though, you need the supervised clinical practice your state requires, and a clinical concentration gives you a clinical field placement to build on.
Which MSW Specialization Is Most Affordable?
By median estimated tuition, medical social work is lowest at $39,000, followed by substance use at $40,020. Child and family and aging share the highest median, $47,700.
Which Specializations Are Offered Online?
All of them. 129 clinical programs run online, as do 59 macro, 48 child and family, 27 medical, 20 school, 17 substance use and 15 aging programs.
Is Advanced Generalist a Specialization?
It counts as the specialized year at schools that use it. Ball State University's advanced generalist track focuses on rural practice with coursework in health, addictions and gerontology, and Bowling Green State University offers advanced generalist tracks in gerontology, healthcare and children and families.
What Is the Difference Between Mezzo and Macro Social Work?
CSWE's standards describe generalist practice at the micro, mezzo and macro levels. In program terms, micro means work with individuals and families, mezzo means groups and organizations, and macro means communities, policy and administration, the level covered by concentrations such as Leadership and Social Change or Community and Organizational Practice.
